Social Prescribing Service
What is Social Prescribing?
Social prescribing is a holistic approach to health care that connects individuals to non-medical resources and activities within their community to improve their overall wellbeing. By addressing social, emotional and practical needs, social prescribing empowers individuals to take control of their health. This innovative process involves referring patients to a range of local services such as fitness programs, art classes, volunteer opportunities and support groups which can alleviate loneliness, reduce stress and enhance quality of life.

How can Social Prescribing help?
This service connects you to activities and services that may improve your health, build social connections, and foster independence. It's designed to offer non-judgemental support, advice, and guidance tailored to your needs.

Social Prescribers offer support and guidance on a wide range of issues.
